Corey Farrell a6e5bae3ef Remove ASTERISK_REGISTER_FILE.
ASTERISK_REGISTER_FILE no longer has any purpose so this commit removes
all traces of it.

Previously exported symbols removed:
* __ast_register_file
* __ast_unregister_file
* ast_complete_source_filename

This also removes the mtx_prof static variable that was declared when
MTX_PROFILE was enabled. This variable was only used in lock.c so it
is now initialized in that file only.

Corey Farrell 478fb4a388 MALLOC_DEBUG: Replace WRAP_LIBC_MALLOC with ASTMM_LIBC.
There are 3 ways that calls directly to standard allocator functions can
be dealt with:
1. Block their use, cause them to generate an error. This is the default.
2. Replace them with the Asterisk equivalent function calls.
3. Leave them alone.

This change allows one of these 3 options to be selected by any source.
The source just needs to define ASTMM_LIBC to ASTMM_BLOCK, ASTMM_REDIRECT,
or ASTMM_IGNORE to use option 1, 2 or 3 respectively. Normally ASTMM_BLOCK
is the correct option, so it is default when ASTMM_LIBC is not defined.
In some cases when building 3rd party code it is desirable to have it use
Asterisk functions, without changing the whole source - ASTMM_REDIRECT
accomplishes this. When using 3rd party libraries sometimes a static
inline function will make use of malloc or free. In these cases it may
be unsafe to replace the allocator in the header, as it's possible the
memory could be freed by the library using standard allocators. For
those cases ASTMM_IGNORE is needed.

Corey Farrell 2d4dc0c963 Fix error's produced by astmm.h when standard allocators are used.
astmm.h includes defines that are meant to cause error's when standard
allocators (malloc, calloc, free, etc) are used. It actually only
causes a warning, which is not always caught on certain sources. In
modules this unknown symbol is not detected until runtime, where the
module fails to load. This modifies the define's so that using one
of the blocked functions will cause a compile error regardless of
CFLAGS.

Moved spandsp header includes to before asterisk.h so the static inline
functions can continue using malloc and free. Although these functions
are never called and optimized away, the updated replacement macro's
would still cause a failure.

Kevin Harwell 5d0c182885 res_fax: allow 2400 transmission rate according to v.27ter standard
A previous set of patches (see: ASTERISK-22790 & ASTERISK-23231) made it so
a v.27 modem was not allowed to have a minimum transmission rate of 2400 bits
per second. This reverts all or some of those patches since according to the
v.27ter standard a rate of 2400 bits per second is also supported.

One of the original patches also added 9600 bits per second support for v.27.
This patch also removes that since v.27ter only supports 2400/4800 bits per
second.

Also, since Asterisk specifically supports v.27ter the enum was renamed to
better reflect this.

Matt Jordan 4a58261694 git migration: Refactor the ASTERISK_FILE_VERSION macro
Git does not support the ability to replace a token with a version
string during check-in. While it does have support for replacing a
token on clone, this is somewhat sub-optimal: the token is replaced
with the object hash, which is not particularly easy for human
consumption. What's more, in practice, the source file version was often
not terribly useful. Generally, when triaging bugs, the overall version
of Asterisk is far more useful than an individual SVN version of a file. As a
result, this patch removes Asterisk's support for showing source file
versions.

Specifically, it does the following:

* Rename ASTERISK_FILE_VERSION macro to ASTERISK_REGISTER_FILE, and
remove passing the version in with the macro. Other facilities
than 'core show file version' make use of the file names, such as
setting a debug level only on a specific file. As such, the act of
registering source files with the Asterisk core still has use. The
macro rename now reflects the new macro purpose.

* main/asterisk:
  - Refactor the file_version structure to reflect that it no longer
    tracks a version field.
  - Remove the "core show file version" CLI command. Without the file
version, it is no longer useful.
  - Remove the ast_file_version_find function. The file version is no
    longer tracked.
  - Rename ast_register_file_version/ast_unregister_file_version to
    ast_register_file/ast_unregister_file, respectively.

* main/manager: Remove value from the Version key of the ModuleCheck
Action. The actual key itself has not been removed, as doing so would
absolutely constitute a backwards incompatible change. However, since
the file version is no longer tracked, there is no need to attempt to
include it in the Version key.

* UPGRADE: Add notes for:
  - Modification to the ModuleCheck AMI Action
  - Removal of the "core show file version" CLI command

Richard Mudgett a8ea2f9287 res_fax.c, res_fax_spandsp.c: Remove redundant locking.
When FAX was developed, apparently the faxregistry.container used to be a
linked list that was converted to an ao2 container. Some of the
replacement ao2 container operations still had explicit lock/unlocks
around them.

Three off nominal code paths in res_fax.c and res_fax_spandsp.c unlock the
channel even though the routine did not lock the channel and other code
paths in the routine do not unlock the channel.

Matthew Jordan a2c912e997 media formats: re-architect handling of media for performance improvements
In the old times media formats were represented using a bit field. This was
fast but had a few limitations.
 1. Asterisk was limited in how many formats it could handle.
 2. Formats, being a bit field, could not include any attribute information.
A format was strictly its type, e.g., "this is ulaw".
This was changed in Asterisk 10 (see
https://wiki.asterisk.org/wiki/display/AST/Media+Architecture+Proposal for
notes on that work) which led to the creation of the ast_format structure.
This structure allowed Asterisk to handle attributes and bundle information
with a format.

Additionally, ast_format_cap was created to act as a container for multiple
formats that, together, formed the capability of some entity. Another
mechanism was added to allow logic to be registered which performed format
attribute negotiation. Everywhere throughout the codebase Asterisk was
changed to use this strategy.

Unfortunately, in software, there is no free lunch. These new capabilities
came at a cost.

Performance analysis and profiling showed that we spend an inordinate
amount of time comparing, copying, and generally manipulating formats and
their related structures. Basic prototyping has shown that a reasonably
large performance improvement could be made in this area. This patch is the
result of that project, which overhauled the media format architecture
and its usage in Asterisk to improve performance.

Generally, the new philosophy for handling formats is as follows:
 * The ast_format structure is reference counted. This removed a large amount
of the memory allocations and copying that was done in prior versions.
 * In order to prevent race conditions while keeping things performant, the
ast_format structure is immutable by convention and lock-free. Violate this
   tenet at your peril!
 * Because formats are reference counted, codecs are also reference counted.
The Asterisk core generally provides built-in codecs and caches the
ast_format structures created to represent them. Generally, to prevent
inordinate amounts of module reference bumping, codecs and formats can be
added at run-time but cannot be removed.
 * All compatibility with the bit field representation of codecs/formats has
been moved to a compatibility API. The primary user of this representation
is chan_iax2, which must continue to maintain its bit-field usage of formats
   for interoperability concerns.
 * When a format is negotiated with attributes, or when a format cannot be
represented by one of the cached formats, a new format object is created or
cloned from an existing format. That format may have the same codec
underlying it, but is a different format than a version of the format with
   different attributes or without attributes.
 * While formats are reference counted objects, the reference count maintained
on the format should be manipulated with care. Formats are generally cached
and will persist for the lifetime of Asterisk and do not explicitly need
to have their lifetime modified. An exception to this is when the user of a
format does not know where the format came from *and* the user may outlive
the provider of the format. This occurs, for example, when a format is read
from a channel: the channel may have a format with attributes (hence,
non-cached) and the user of the format may last longer than the channel (if
the reference to the channel is released prior to the format's reference).

For more information on this work, see the API design notes:
  https://wiki.asterisk.org/wiki/display/AST/Media+Format+Rewrite

Finally, this work was the culmination of a large number of developer's
efforts. Extra thanks goes to Corey Farrell, who took on a large amount of the
work in the Asterisk core, chan_sip, and was an invaluable resource in peer
reviews throughout this project.

There were a substantial number of patches contributed during this work; the
following issues/patch names simply reflect some of the work (and will cause
the release scripts to give attribution to the individuals who work on them).

Sean Bright 8357027080 res_fax_spandsp: Use g711_free() when available.
Per Johann Steinwendtner on the asterisk-dev mailing list:

http://lists.digium.com/pipermail/asterisk-dev/2014-March/066102.html

g711_free() was introduced in spandsp 0.0.6pre4 and g711_release() became a
noop. I opted not to remove the call to g711_release() since it is harmless
and to call g711_free() if we have a sufficiently recent version of spandsp.

Matthew Jordan 38da8d2fda res_fax_spandsp: Fix crash when passing ulaw/alaw data to spandsp
When acting as a T.38 fax gateway, res_fax_spandsp would at times cause a crash
in libspandsp. This would occur when, during fax tone detection, a ulaw/alaw
frame would be passed to modem_connect_tones_rx. That particular routine
expects the data to be in slin format. This patch looks at the frame type and,
if the data is ulaw/alaw, converts the format to slin before passing it to
modem_connect_tones_rx.

Matthew Jordan 8042f4cdd2 res_fax_spandsp: Always init T.38 session to avoid crashes during state change
Prior to this patch, res_fax_spandsp was conservative with how it initialized
the spandsp T.38 context. It would only initialize it if the driver thought
the current state was a T.38 fax. While this works fine in nominal situations,
in certain off nominal situations, res_fax_spandsp can believe that a T.38
fax will not occur when in fact one has started. In particular, this was
discovered when res_fax would fall back to audio after timing out on a T.38
upgrade. The SIP channel driver would continue to retry the re-INVITE and -
if the remote end responded after res_fax timed out with a 200 OK - a T.38
frame would be delivered to the res_fax stack when it no longer expected it.

As it turns out, there does not appear to be any downside to always
initializing the T.38 context, other than the actual memory allocation.
Since that avoids this off nominal situation (and others which are equally
likely hard to predict), this is the safest way to avoid this problem.

Much thanks to Torrey as well for providing a scenario that reproduces this
issue.

Matthew Jordan 2d2a47fae3 Fix some more fax test errors due to needing the peer in a bridge
In r389799, a number of fax errors in gateway mode were fixed by using the
appropriate function to get a channel's peer while in a bridge. This patch
does two things:
(1) It uses the same function in res_fax_spandsp while starting the fax
gateway. Without this, the fax gateway will not actually start up, as
res_fax_spandsp also must inspect the channel's peer in a two-party
    bridge
(2) It refactors some ao2 objects in sendfax_exec to use RAII_VAR. This was
reverted in r389799 as some off nominal paths were getting hit without
the fix in (1) that indicated an ao2 object issue; this turned out to
be a red herring (which is an odd phrase)

Matthew Jordan a0c363e227 Refactor ast_timer_ack to return an error and handle the error in timer users
Currently, if an acknowledgement of a timer fails Asterisk will not realize
that a serious error occurred and will continue attempting to use the timer's
file descriptor. This can lead to situations where errors stream to the
CLI/log file. This consumes significant resources, masks the actual problem
that occurred (whatever caused the timer to fail in the first place), and
can leave channels in odd states.

This patch propagates the errors in the timing resource modules up through
the timer core, and makes users of these timers handle acknowledgement
failures. It also adds some defensive coding around the use of timers
to prevent using bad file descriptors in off nominal code paths.

Note that the patch created by the issue reporter was modified slightly for
this commit and backported to 1.8, as it was originally written for
Asterisk 10.

Kevin P. Fleming fdda494776 Improve T.38 gateway V.21 preamble detection.
This commit removes the V.21 preamble detection code previously added to the
generic DSP implementation in Asterisk, and instead enhances the res_fax module
to be able to utilize V.21 preamble detection functionality made available by
FAX technology modules. This commit also adds such support to res_fax_spandsp,
which uses the Spandsp modem tone detection code to do the V.21 preamble
detection.

There should be no functional change here, other than much more reliable V.21
preamble detection (and thus T.38 gateway initiation).

David Vossel c26c190711 Asterisk media architecture conversion - no more format bitfields
This patch is the foundation of an entire new way of looking at media in Asterisk.
The code present in this patch is everything required to complete phase1 of my
Media Architecture proposal. For more information about this project visit the link below.
https://wiki.asterisk.org/wiki/display/AST/Media+Architecture+Proposal

The primary function of this patch is to convert all the usages of format
bitfields in Asterisk to use the new format and format_cap APIs. Functionally
no change in behavior should be present in this patch. Thanks to twilson
and russell for all the time they spent reviewing these changes.

Matthew Nicholson 13f523731a Update res_fax and res_fax_spandsp to be compatible with Fax For Asterisk 1.2.
The fax session initilization code for T.38 faxes has been rewritten. T.38 session initialization was removed from generic_fax_exec, and split into two different code paths for receive and send. Also the 'z' option (to send a T.38 reinvite if we do not receive one) was added to sendfax.

In the output of 'fax show sessions', the 'Type' column has been renamed to 'Tech' and replaced with a new 'Tech' column that will report 'G.711' or 'T.38'.

Control of ECM defaults has been added to res_fax

A 'fax show settings' CLI command has been added.

Support of the new AST_T38_REQUEST_PARMS control method request to handle channels that have already received a T.38 reinvite before the FAX application is start has been added.

Support for the 'fax show settings' command has been added to res_fax_spandsp and handling of the ECM flag has been slightly altered.
