As part of moving the reference manual to a real reference manual I
am removing the "appendix" classification and having everthing be
a chapter. I changed the "ref-imagese" and "ref-features" appendices
to chapters and made any references in the doc set use the proper
term now (chapter).

Previous renditions of the term "Build Appliance" referred to the
VMware system. However, it was determined that the term
"Build Appliance" should really refer to the actual image that
can be booted and run on the VMware applications. I have updated
the areas affected by this change. The change assumes the name of
the built or downloaded file you boot will be named
"build-appliance-image."

Three cross-references to the section "External Development
Using the Meta-Toolchain" were removed. This section is going
away in the YP Reference Manual. There is no replacement section
that makes sense so the cross-reference is removed.

Paul sent me a bunch of minor edits and technical corrections
for the ADT, BSP, Development, and YP reference manual.
Most changes were taken as he submitted them. However, some
formatting conventions were not followed in the commits he made
to yocto-docs/paule/docs2. After merging them to my yocto-docs/master
branch I fixed some issues.

-live and -directdisk image have been removed; add a tip about using the
live option in IMAGE_FSTYPES to build any image as a live image.

[BUGID# 873] - Added a note in the Images Appendix indicating that
building an image without GPLv3 components is only supported for
base and minimal images. Also put the two changes you have to do
to the local.conf file for the build.
Added a note in the second chapter in the section on building images.
The note indicates the same as in the appendix but does not go into the
local.conf file detail.

Added command 'ls meta*/recipes*/images/*.bb' as the command to see the
supported images. Also added poky-image-lsb as an image and noted
that poky-image-sdk has becom poky-image-sato-dev.
These fixes are in response to alpha testing for release 1.0 Yocto.

Cyril submitted a patch via email form for several spelling corrections. The master docs, however, are not up-to-date with what I have on my contrib area. Many of Cyril's corrections were already addressed. A few were not and I have updated them here with this commit.

In the Ref Images appendix there was a link to the section
"External Development Using the Poky SDK" section that did not work
due to the section id string changing. I fixed the link.
